Ok -- this is what gets you scheduled to take the Exit Interview -- in other words, if you don't fill this out, your Exit Interview will not happen -- this is what "triggers" the phone call -- so be sure you give us the best phone number for reaching you -- VERY IMPORTANT.

If you're not available when the call comes through, call or text the interviewer at the number they leave on your voice mail. The interviewer will also send you a text or email to try and reach you if necessary.
